Why it matters

TBG is the main protein carrying thyroid hormones in the blood. Because bound hormone isn't active, TBG levels affect the relationship between total and free hormone measurements.

It's measured when total and free thyroid results don't line up as expected. Pregnancy, oestrogen therapy and some inherited variations affect TBG.
