Why it matters

Total testosterone measures everything circulating — both the portion bound to carrier proteins and the small fraction that's free and available to tissues.

Because most is bound, the total on its own can be misleading. SHBG or a free testosterone measurement alongside is what makes it interpretable.

Before you collect

Morning collection, ideally before 10am. Testosterone follows a daily rhythm and falls through the day. Recent illness and heavy training also affect results.
