Why it matters

SHBG is the protein that binds sex hormones in the blood. Hormone bound to SHBG isn't available to tissues, so SHBG determines how much of a given total is actually usable.

That's why a total testosterone without SHBG can be misleading — the same total means different things at different SHBG levels.

SHBG itself is affected by thyroid status, liver function, insulin and some medications.
