Why it matters

Most testosterone circulates bound to SHBG and isn't available to tissues. FAI estimates the proportion that is, by comparing total testosterone against the amount of binding protein present.

It's a calculation rather than a direct measurement, which means it inherits the limitations of both inputs — but it's more informative than a total testosterone on its own.
